Device Protection Packages

1. Device Protection Service
The Device Protection Service package combines the benefits of the Device Coverage and Device Enhanced Warranty programs. Your device and certain standard accessories (if part of the covered loss) are covered against loss, theft, liquid or physical damage and protected against mechanical or electrical failures due to defects in materials or workmanship or normal wear and tear.
- Devices:
- $6.99 per month per covered device
- A $50 or $100 non-refundable deductible/Claim Service Fee (depending on model) applies per approved claim. For a list of equipment in the program, please visit www.phoneclaim.com/ntelos or call Asurion at (877) 868-8775.
- Maximum replacement value of $1,000 per occurrence.
- iPhones:
- $10.99 per month per covered device
- $170 non-refundable deductible (iPhone 4) applies per approved claim.
- $200 non-refundable deductible (iPhone 4S, iPhone 5) applies per approved claim.
- Maximum replacement value of $1,000 per occurrence.

- Three occurrences within any consecutive 12 months for loss, theft, damage, and extended warranty claims with a maximum replacement value of $1,000 per occurrence.
- Replacement devices may be new or remanufactured and/or a comparable model.
- Devices are only eligible for enrollment on the date of new activation or upgrade.
2. Device Coverage
The Device Coverage package offers insurance coverage for your wireless device and certain standard accessories (if part of the covered loss). If you have Device Coverage and your device is lost, stolen or accidentally damaged (including liquid and physical damage), you may file your claim with Asurion within 60 days of the incident and the device will be replaced upon claim approval, typically the next day. Device Coverage is part of Device Protection Service and is also sold separately.

- $5.39 per month per covered device

- Maximum replacement value of $1,000 per occurrence
- iPhones:
- $8.39 per month per covered device.
- $170 non-refundable deductible (8GB, iPhone 4) applies per approved claim.
- $200 non-refundable deductible (16GB, 32GB, 64GB iPhone 4S) applies per approved claim.
- Maximum replacement value of $1,000 per occurrence.
- Three occurrences within any 12 months with a maximum replacement value of $1,000 per occurrence.
- Asurion may fulfill claims with new or remanufactured equipment. In general, claims are fulfilled with the same make and model you claim. If your make and model is not available, a like-kind and quality make and model will be substituted.
- Color, feature and accessory compatibility are not guaranteed.
- Devices are only eligible for enrollment on the date of new activation or upgrade.
3. Device Enhanced Warranty
The Device Enhanced Warranty package provides coverage against mechanical or electrical defects after the manufacturer’s warranty expires. The monthly cost is $2.99 per device. Device Enhanced Warranty is part of Device Protection Service and is also sold separately.
- Devices:
- $2.99 per month per covered device (only $1.60 when purchased as part of Device Protection Service)
- A $50 or $100 non-refundable deductible (depending on model) applies per approved claim. There is no deductible for approved malfunctions during the manufacturer’s warranty period. How To Enroll
For any of these programs, nTelos Wireless customers can only enroll at activation:
- New account activation with new device.
- Upgrade with device purchase.
- To enroll in any of these programs, customers should visit any nTelos Wireless retail store, visit nTelos online, or call (877) 468-3567.
How to File A Claim
Warrantable Failure:
If you are enrolled in either Device Protection Service or the Device Enhanced Warranty program, visit any nTelos Wireless retail store to diagnose and address the problem with your wireless device during the term of the original warranty.
Lost, theft and damage or failures of your device after the expiration of your original warranty:
- Call 877-868-8775 or visit www.phoneclaim.com/ntelos.
- If your device is lost or stolen, contact nTelos Wireless immediately at (877) 468-3567 to suspend your service.
- File your claim within 60 days of loss.
- Pay a $50 or $100 non-refundable deductible depending on device model per approved claim.
- If your device is damaged or later recovered, it must be returned to the authorized service center, to avoid the non-returned equipment charge of up to $325.
- Contact Asurion at (877) 868-8775 and they will mail you the necessary shipping material to return the device and avoid the charge.
Cancelling Coverage
- Cancellation Policy: You may cancel your optional coverage at any time and receive a pro-rated refund of your unearned/unused charges.
- Customers wishing to change their coverage in My Account should not remove the feature and should contact nTelos Wireless Customer Care at (877) 468-3567 for assistance.
